The Grenfell Tower Inquiry was set up to examine the circumstances leading up to and surrounding the tragic fire at Grenfell Tower on 14 June 2017 in London, England in which 72 people died. In an occurrence like this where many parties are involved in a catastrophe the “Web of Blame” appears to be one of the few techniques that graphically describe the scope of the systemic failure. As the lead counsel Richard Millett KC said . “Expressions of regret for the victims of the fire have been as common, to the point of trite, as admissions of responsibility have been rare.” The link to the report is below. It seems to me that this is one of the best investigative reports I have read into a catastrophic situation.
Additionally this technique is a brutal way of highlighting the blame-shifting and finger pointing that typically accompany investigations into catastrophic failures (or maybe even just everyday failures).
I can think of some politicians who might benefit from this technique!
